Admission Criteria

• Approval received from Indiana Pre-Admission Screening Program (Real Services). Contact your local Area on Aging, (574) 284-2644 with questions.
• Physical examination with a current history and physical that has been completed within the last 30 days.
• A current Mantoux test that has been completed within the last 30 days.
• A current chest x-ray that has been completed within the last six (6) months.
• A physician that has been selected, who agrees to follow the care of the future resident in our facility. A list of physician affiliations can be provided per request.
• The resident must be approved for admission by St. Joseph's Care Center's assessment team.
• Completion of all admission paperwork prior to admission and copies of social security card, Medicare card, and Insurance cards as well as Living Will and Power of Attorney documentation.
