Admissions

Admission Requirements

  • History and Physical exam (no more than 5 days old)
  • Physician’s orders for admission to Sanctuary at St. Joseph's
  • Results of a Chest X Ray (no more than six months old)
  • Results of a TB test (no more than 30 days old)
  • Health insurance cards (Medicare, Medicaid, Blue Cross etc.)
  • Social Security card

A resident cannot be admitted from home without all necessary medical forms. To assure a current History and Physical, an appointment with the patient’s attending physician may be required.

Your family member will be assigned to a physician prior to admission. If a resident’s attending / family physician wishes to be responsible for following the resident at Sanctuary at St. Joseph's, arrangements MUST be made prior to admission.

Federal and Indiana law and rules, require that the resident’s attending physician see the resident within 48 hours of admission, every 30 days for the first 90 days and a minimum of every 30 to 60 days thereafter.
