An additional benefit without any cost or obligation to our residents and families is our Case Management Service. Our Case Manager is a Registered Nurse that works in our hospitals to assess residents who require the services of our nursing facilities.

Our Case Manager also works closely with the Director of Nursing and the Admission Director in our facilities to determine and manage placement needs while maintaining the confidentiality of all resident care information.

Services included but not limited to include:

• Provide pre-admission screening in the hospital, home, or care setting
• Comprehensive personal assessment of each resident's needs and preferences
• Complete and thorough assessment
• Available to interpret resident rights to the resident, family and staff personnel as necessary or appropriate
• Consideration and assessment for physical, occupational and speech therapies
• Works closely with the physician and family
• Provides a professional and personal interest in each case with the focus on positive results
• Works with patients, families and insurers on each patient’s plan of discharge to the facility of choice
• Targets the essential medical and discharge planning objectives necessary to achieve a safe and successful discharge
